Documents Required:
1. Print out of online application.
2. Admit Card issued by ESIC for appearing in the Online Test
3. Educational Qualification Certificates.
4. Govt. approved Photo Identity Documents Viz- Adhaar Card, Pan Card, Driving License, Voter Card, Passport etc
5. Age proof Certificate.
6. Caste certificate(SC/ST/OBC) issued by appropriate authority (in case of OBC category, latest OBC Certificate in prescribed proforma along with Form of Declaration as upload with the above Result. The Candidate must bring the latest OBC certificate in the prescribed Format issued by the Competent Authority.)
7. Disability Certificate in case of Person with Disability(PWD).
8. Discharge Certificate in case of Ex-SM.
9. The Candidate already working in Govt. Office/PSU/Autonomous body/Statutory body is required to produce No Objection Certificate(NOC) from his/her parent department at the time of Verification.

How to Apply :
1. The candidates shortlisted for verification must bring all original documents in support of their eligibility criteria for the respective posts at the time of verification along with one set of self-attested copies of the documents. The following documents are invariably required at the time of Document verification
2. Calling of candidates for Document Verification does not confer any right upon the candidate for his/ her appointment for the concerned post.
3. The candidature of candidates is purely provisional. If at any stage, it is found that he/ she is involved in any malpractices in the examination process, his/her selection/appointment shall be summarily cancelled and appropriate legal action shall be initiated against him/her.
4. Candidates will have to make their own arrangement of stay for appearing in the Document Verification Process and no TA/DA is admissible for the same.
